We're about to have our resources moved from one Subscription to an other. (new subscription ID, same tenant) Are we able to just to a find and replace on the state file on the old sub ID with the new sub ID?

Yes, I don't see why that wouldn't work. Be sure to use a recent version of the Azure Native provider.
