~do stack names have to be unique across the org/team?~ yeah they do

We have a work item to reconsider this and make stack names unique only per project -- and I fully expect we will do it. <https://github.com/pulumi/pulumi/issues/2039> tracks this and we'll be considering it soon, feedback appreciated.
